Does Ozempic Cause Hair Loss? What the Research Shows

How common GLP-1 hair loss really is, why it happens, who is most affected, how long it lasts, and what helps — drawn from a 2026 review of 24 studies.

Educational, not medical advice. Sudden patchy hair loss, scalp pain or redness, or hair loss alongside other new symptoms should be checked by a clinician or dermatologist.

Hair in the shower drain is one of the most-searched GLP-1 side effects, and one of the most unsettling. The research has good news: GLP-1 hair loss is usually temporary, it is closely tied to how fast weight comes off, and there are practical ways to reduce it.

How common is hair loss on GLP-1s?

More common than on placebo, but still a minority of people. In the Wegovy trials, 3.3% of adults on semaglutide 2.4 mg reported hair loss versus 1% on placebo. A 2026 systematic review of 24 studies found tirzepatide users reported hair loss at 5.4% versus 0.9% on placebo, and an overall rate of about 6 cases per 1,000 patient-years on GLP-1s versus under 1 on placebo.

Why do GLP-1s cause hair loss?

Most GLP-1 hair loss is telogen effluvium — a temporary shift of hair follicles into their resting, shedding phase after a stress on the body. Rapid weight loss, a sharp drop in calories, and low protein are classic triggers. That is why the pattern tracks weight loss: in the review, people who lost more than 20% of their body weight reported hair loss about twice as often (5.3% versus 2.5%) as people who lost less.

In the research, shedding typically appears about two to three months after starting treatment.

Who is most likely to lose hair on a GLP-1?

  • Women. In the Wegovy trials, 4% of women reported hair loss versus 0.9% of men. In tirzepatide trials the gap was 7.1% versus 0.5%.
  • People losing weight fastest, especially past about 20% of body weight.
  • People on higher doses. The review found semaglutide's association was dose-dependent: weight-loss doses were implicated far more often than doses under 2 mg a week.
  • People eating too little protein while appetite is suppressed — nutrition is one of the few levers you control.

Is GLP-1 hair loss permanent?

Usually not. The review describes it as typically self-limited and reversible, with improvement generally seen 3–6 months after weight stabilizes, and found no strong evidence of permanent follicle damage. A smaller share of reports involved androgenetic (pattern) hair loss, which behaves differently — one reason persistent or patterned thinning deserves a dermatologist's look.

How can you prevent hair loss on a GLP-1?

  • Put protein first. Build each meal around adequate protein while your appetite is suppressed.
  • Cover your micronutrients. The review flagged iron, zinc, vitamin D, vitamin B12, and vitamin A. Ask your clinician before starting supplements, and whether lab checks make sense for you.
  • Don't rush the dose. Gradual dose increases slow the pace of weight loss, and gradual escalation was one of the review's counseling recommendations.

Adequate protein is the first nutrition priority the 2026 review recommends during GLP-1 weight loss.

When should you see a doctor about hair loss?

The review suggests a dermatology referral if shedding lasts beyond 6–9 months or follows a pattern, such as a widening part or a receding hairline. See someone sooner for patchy bald spots, scalp redness or pain, or hair loss alongside fatigue or other new symptoms, which can point to thyroid, iron, or other causes worth checking.

Common questions about GLP-1 hair loss

Does Ozempic cause hair loss?
Hair loss is more common on semaglutide — the active ingredient in Ozempic and Wegovy — than on placebo: 3.3% versus 1% in the Wegovy weight-loss trials. It is usually temporary telogen effluvium linked to rapid weight loss.
Do Mounjaro and Zepbound cause hair loss?
Tirzepatide users reported hair loss at 5.4% versus 0.9% on placebo in a 2026 review of the evidence, with women affected far more than men (7.1% versus 0.5% in tirzepatide trials).
Will my hair grow back after GLP-1 hair loss?
In most cases, yes. GLP-1 hair loss is usually telogen effluvium, which the 2026 review describes as typically reversible, with improvement 3–6 months after weight stabilizes. See a dermatologist if shedding lasts beyond 6–9 months or looks patterned.
How do I stop hair loss on a GLP-1?
Eat adequate protein, cover key nutrients such as iron, zinc, and vitamin D (ask your clinician before supplementing), and avoid rushing dose increases, since faster weight loss is linked to more shedding.

Bottom line

Hair loss on GLP-1s is real but usually temporary, and it tracks the speed of weight loss more than anything else. Prioritize protein and nutrients, don't rush dose increases, and see a dermatologist if shedding lasts beyond 6–9 months or looks patterned. This is educational, not medical advice.
